What we do hasn’t changed
The pan-London homeless health programme led by Transformation Partners in Health and Care becomes the Homeless Health London Partnership. This new brand marks a fresh start while reinforcing the programme’s commitment to its core mission. That is to improve the access to, experience of, and outcomes from health care for people experiencing, or at risk of, homelessness.
What we do and why we do it has not changed, but the Homeless Health London Partnership was developed to consolidate programme activities under a cohesive look and feel and a more consistent voice.
A better reflection of partnership working
We work in tandem with the five NHS London integrated Care Boards, regional statutory health bodies, local government, third-sector organisations and people with lived experience to address challenges impacting the health of people experiencing homelessness.
The new brand name reflects this partner coalition of health, housing, and social care stakeholders to break down barriers to healthcare. We believe that by collaborating and co-producing solutions together, we can influence better and lasting changes within these sectors.
We are proud to be part of the NHS and we are committed to advocating for compassionate and effective healthcare services that are made with the community, for the community.
