Social prescribing gains traction at NHS Expo 2018

Healthy London Partnership’s Proactive Care Team attended the NHS Expo conference in Manchester 5-6 September and hosted a ‘sofa conversation’ about social prescribing along with colleagues from NHS England.

It was a good opportunity to meet people from around the country who are similarly passionate about social prescribing and tell them about the great work going on across the five sustainability and transformation partnerships (STPs) in London. Matt Hancock, Secretary of State for Health and Social Care, endorsed the expansion of social prescribing in his keynote address at the conference.

Expo coincided with the launch of two new short films we have produced in partnership with Merton CCG to showcase their Social Prescribing scheme that has been successfully piloted in 2 GP practices with plans to spread to a total of 9 practices across East Merton this year.

This film captures stories from patients who have been linked up with support and activities in their local communities by the social prescribers and the difference it has made to their health and sociality.  This film provides useful advice from all the key partners involved about how to set up and deliver a social prescribing connector scheme within primary care.
