Meaningful involvement

Actions to take

  • Explore, with service users, why they may have increased health risks, what specific factors in their lives are contributing to these risks and what they feel would be helpful ways to address them.
  • Ensure care planning promotes self-management and holistic wellbeing support, addressing the broader lifestyle factors contributing to poor health
