Connecting Care for Children (CC4C) is an innovative model of healthcare that aims to provide different levels of healthcare in the community setting. Primary, secondary and tertiary healthcare professionals assemble in GP hubs. The aim is to deliver early access to specialist child medical knowledge for children and young people (CYP) in a local GP setting. As such, CYP can access such care from the comfort of the familiar GP practice to avoid the often daunting experience of going to hospital.


In this section of the toolkit, the resources include a service evaluation of Child Health General Practice Hubs describing the applied concept of GP Hubs and the resulting benefits for both patients and medical professionals. A table summarises the value, outcomes and benefits of the CC4C model.
