A summary of the recommended interventions identified in NHS England’s Consolidated Channel Shift Model.
In February 2017, NHS England published a suite of urgent and emergency care interventions called the Consolidated Channel Shift Model. This is a model designed for system leaders to respond to increasing demand in health and care. It will be of interest to commissioners and providers as they prepare for the increase in demand during winter.
This document has been shared with London Sustainability and Transformation Planning Leads, Programme Directors, Chief Officers, Chief Finance Officers and Chairs.

- Delivering care in the community
- Reducing inappropriate admissions
- Avoiding hospital admission
- Reducing time spent in hospital
