The Montefiore Health System in New York: integrated care and the fight for social justice The Montefiore Health System in the Bronx, New York, has found ways of helping even the most deprived by stepping beyond the bounds of conventional health services. This commissioned report builds on the overview of the Montefiore Health System we published in April 2018 and explores building primary and community care, managing patients with complex needs, supporting infrastructure, and elements of a high-performing system. The lives we want to lead: the LGA green paper for adult social care and wellbeing The Local Government Association (LGA) has launched its own green paper setting out how the adult social care system could be improved and the radical measures that it believes need to be considered given the scale of the funding crisis. The LGA is seeking the views of people and organisations from across society on how best to pay for care and support for adults of all ages and their unpaid carers and aims to make the public a central part of the debate. Responses to this consultation should be made by 26 September. Delivering integrated care: the role of the multidisciplinary team This report from the Social Care Institute for Excellence outlines its integration logic model and highlights the role of multidisciplinary teams (MDTs) as a means to enable practitioners and other professionals in health and social care to collaborate successfully. Sufficient diversity of professions and disciplines, suitable leadership and team dynamics, and supportive organisations are important enablers. In good health: a report following the All Party Pharmacy Group’s 2018 inquiry into long term conditions This report highlights the opportunities for community pharmacies to deliver and improve care for patients with long-term conditions. It outlines four key actions that would enable long-term conditions to be managed in a community setting and argues that these actions should be implemented by 2020. IHI innovation system This Institute for Healthcare Improvement paper describes how a healthcare organisation might create its own internal innovation system, based on the needs of the organisation, that focuses on improving health care delivery. It uses examples from IHI’s 30 years of experience with innovation to highlight how to move through this process, determine what is right for an organisation, and balance innovation activities with ongoing operations.
