Demand for health and social care is increasing as people live longer with complex conditions. A workforce modelling tool is a useful addition for those wishing to bring about new models in primary and integrated care, and service re-design around specific groups of patients.

City and Hackney neighbourhood model

Dr Stephanie Coughlin, clinical lead for neighbourhoods, and Jennifer Walker, neighbourhood development programme lead, have developed a case study which details the progress and findings made within City and Hackney Clinical Commissioning Group (CCG).

City and Hackney’s Neighbourhood Model highlights work being done to empower and support residents in their independence, while strengthening communities and networks to support individuals where required. The model aims to bring about a shift in the culture of how people approach health and wellbeing, making it more person-centred and allowing residents and patients to build more personal resilience, increased confidence in self-management as well as addressing their health and social needs.
