The national personalised care team and Healthy London Partnership – proactive care team are jointly hosting the Personalised and Proactive Care London forum on 15 October at 9.30am – 3.30pm.
We are working together to support the five London STPs to embed personalised and proactive care. Personalised Care is based on Integrated Personal Commissioning (IPC) as the main vehicle, with six additional components: personal health budgets; personalised care and support planning; patient activation; social prescribing; shared decision making; and patient choice.

The event invites London stakeholders (STPs, CCGs, NHS providers, local authority representatives, VCS etc.) to come together to learn more about how to effectively implement personalised care at scale and pace and deliver benefits to patients, professionals and the NHS and care system.
There will be opportunities to learn from those who have had success in implementing personalised care strategies as well as to network and share with colleagues within your local area working across the personalised care agenda. Register here.
